It doesnt matter if you're ip banned or not, you gotta follow the rules for a ban appeal or else your ban won't be lifted. In other words, you have to either:

A) Make a new topic following those guidelines that Josh gave you.
or
B) Edit your first post to include the answers to those 3 questions.
